This Washington Consumer Health Privacy Policy (“WA Policy”) applies to information defined as “Consumer Health Data” under the Washington State My Health My Data Act (“MHMD”), relating to consumers residing in Washington and consumers whose Consumer Health Data is collected in Washington (herein referred to as “Washington Consumers”, “you”, or “your”).
CONSUMER HEALTH DATA WE COLLECT Our Website collects information that identifies, relates to, describes, references, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular consumer, household, or device. MHMD CONSUMER RIGHTS MHMD provides certain rights to Washington Consumers with respect to their Consumer Health Data, subject to certain exceptions. You have the right to access and request deletion of your Consumer Health Data we may have collected, shared, or sold. If you provided consent for collection or sharing of your consumer health data, you have the right to withdraw your consent. In addition, you have the right to appeal our refusal to act on your request. If your appeal is denied, you may submit a complaint with the Washington State Attorney General at www.atg.wa.gov/file-complaint.
